Job summary

Adaptive Home Health, LLC in Texas is seeking an Occupational Therapist (OT) for field-based, full-time work delivering in-home evaluations and skilled visits. This role requires a Texas OT license, strong clinical judgment, willingness to travel daily across the assigned territory, and excellent documentation.

Qualifications

• Active Texas OT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education).

Responsibilities

• Complete in‑home OT evaluations and deliver skilled OT visits.
• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
• Recommend and train on adaptive equipment and home modifications as needed.
• Coordinate with nursing and the broader care team when condition or needs change.
• Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
