You will develop clear plans of care centered on ADLs/IADLs, safety and fall prevention, and the use of adaptive equipment. Travel within assigned territory is required; collaboration with nurses and the broader care team is essential.

Qualifications

• Active Texas OT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Ability to escalate changes in condition to RN/MD with clear rationale.
• Strong communication and documentation skills.
• Driver's license and comfort with daily travel across assigned territory.

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits.
• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
• Recommend and train on adaptive equipment and home modifications as needed.
• Coordinate with nursing and the broader care team when condition or needs change.
