Job summary

Giving Home Health Care, LLC in Santa Fe, NM is seeking a compassionate Nurse (RN or LPN) for in-home patient care on a 1099 contract basis. Visits are 6 hours per week, Thursday, at the patient’s residence, with care guided by a physician-approved plan.

Qualifications

• Graduate of an accredited college or university with an RN/LPN degree.
• Active State or Compact RN/LPN license required.
• Current CPR certification is mandatory.
• TB test and a recent physical examination completed.
• Must maintain required professional insurance.

Responsibilities

• Execute physician-prescribed plans of care.
• Complete compliant documentation in the system.
• Manage and administer prescribed medication and therapies.
• Conduct patient assessments and coordinate care.
• Assist with activities of daily living and monitor vitals.
• Communicate with family, physicians, and case managers.
• Collaborate with CNAs and HHAs to support patients.
