Job summary

Giving Home Health Care is seeking a compassionate Nurse (RN or LPN) to administer patient care in Hernandez, NM under a physician care plan. This 1099 contract role involves home-based visits with 12-hour shifts and a schedule spanning Sun–Sat.

You will collaborate with CNAs, HHA’s, case managers, and physicians to deliver high-quality, patient-centered care. The ideal candidate has nursing licensure, CPR, and the ability to lift up to 50 lbs, with strong communication skills and reliability

Qualifications

• Graduate of an accredited nursing program with RN or LPN licensure.
• Current state or compact license as RN or LPN.
• Current CPR certification.
• TB test and physical examination.
• Maintain required insurance.
• Good interpersonal skills.
• Ability to lift up to 50 lbs and move patients.
• Willing to commute to patient homes.
• Ability to provide basic medical management for medically fragile end-of-life care.

Responsibilities

• Execute physician prescribed plans of care.
• Complete compliant documentation in the system of record.
• Manage and administer prescribed medications, treatments, and therapies.
• Conduct patient assessments and coordination of care.
• Perform activities of daily living.
• Monitor vitals and GI intake/output.
• Assess skin integrity and administer wound care.
• Monitor changes in patient condition and progression.
• Communicate with family, physicians, case managers, and others involved in care.
• Provide education and training to patient, family, and home health aides.
• Work one-on-one in the patients home to ensure comfort.
• Collaborate with and supervise CNAs and PCAs.
• Perform other personal care services as needed.
