Job summary

Qualifications

• Graduate of an accredited nursing program with RN or LPN license.
• Current CPR certification.
• TB test and physical examination completed.
• Own transportation and ability to commute to patient homes.
• Ability to lift up to 50 lbs.

Responsibilities

• Execute physician prescribed plans of care.
• Complete documentation in system of record.
• Manage and administer prescribed medication and therapies.
• Conduct patient assessments and coordination of care.
• Monitor vitals and wound care as needed.
• Collaborate with CNAs, HHAs, case managers, and physicians.
• Provide education to patient and family.
