Job summary

Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to provide in-home health evaluations. The visit lasts about 45 minutes, including history review, physical evaluation and optional diagnostics.

The clinician will travel locally across licensed states and may conduct virtual visits as needed, using an iPad and a clinical workflow. Active, unrestricted NP or PA license with board certification is required; multi-state licenses are preferred.

Responsibilities

• Build trusted relationships with health plan members to improve health outcomes
• Visit millions nationwide to provide connections to the right care
• Work with Signify Health as a clinician colleague
• Be flexible to travel locally and within licensed states
• Conduct virtual visits as needed
• Provide Diagnostic & Preventive Services as indicated
