Job summary

Optum Home & Community Care is hiring a Nurse Practitioner to deliver in-home care across assigned areas. You will perform comprehensive home visits, assess health status, coordinate with interdisciplinary teams, and provide patient and caregiver education. Travel to patient homes is required.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Unrestricted licensure in the practicing state and DEA licensure/prescriptive authority

Responsibilities

• Conduct comprehensive in-home assessments and document findings
• Manage patients through acute events or chronic condition exacerbations
• Provide counseling and education to patients and caregivers
• Collaborate with interdisciplinary care team
• Engage in advanced illness and end-of-life discussions

Education
