UnitedHealth Group is hiring a Nurse Practitioner for in-home care in Georgia, travel required to patient homes within assigned area. The role includes conducting in-home assessments, managing acute and chronic conditions, and coordinating with an interdisciplinary care team to deliver preventive care at home.

A $10,000 sign-on bonus and comprehensive benefits are offered. Active NP certification, an unrestricted license in state of practice, DEA licensure, and access to reliable transportation

Qualifikationen

• Active NP certification (ANCC/AANP) for practice as a Family/Adult/Geriatric/APC.
• Unrestricted licensure in the practicing state or ability to obtain by start date.
• Current DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to establish a collaborative practice agreement where required.
• Reliable transportation for daily in-home visits.

Aufgaben

• Perform comprehensive in-home assessments including history, vitals and exams.
• Manage acute events and chronic condition exacerbations in home settings.
• Support short-term sub-acute rehab needs for members at home.
• Maintain accurate documentation including ICD-10 codes.
• Collaborate with interdisciplinary care team and educate patients/caregivers.
• Engage in advanced illness planning discussions and counseling.
