Job summary

UnitedHealth Group is seeking a clinician to perform in-home assessments, manage acute events, and coordinate care for patients in assigned areas. This role requires active NP/PA certification, state licensure, and valid driver's license.

Daily travel to patient homes is required, with a $10,000 sign-on bonus for external candidates. We offer competitive compensation ranging from $109,500 to $164,000 annually, plus a comprehensive benefits package and 401(k) plan.

Qualifications

• Active NP/PA certification and full state licensure required.
• PA must hold NCCPA certification and state licensure.
• DEA licensure or ability to obtain per state rules.

Responsibilities

• Conduct in-home assessments including medical history, vitals, and exams.
• Perform screenings and point-of-care testing as appropriate.
• Manage acute medical events and chronic condition exac­erations.
• Coordinate care with interdisciplinary team and document ICD-10.
• Provide counseling and education to patients and caregivers.
• Engage in advanced illness conversations and care planning.
