Job summary

UnitedHealth Group seeks a Nurse Practitioner or Physician Assistant to provide in-home care, performing comprehensive assessments and managing acute or chronic conditions at patients' homes. Travel within the assigned area is required, with no nights, weekends, or holidays.

A $20,000 sign-on bonus is offered for external candidates. Active licensure and DEA, plus state requirements, are needed to practice.

Qualifications

• Active ANCC or AANP certification required for NPs.
• State licensure without current disciplinary actions.
• Driver's license and reliable transportation.
• DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to gain a collaborative practice agreement, if applicable.

Responsibilities

• Conduct in-home assessments including medical history, meds reconciliation, vitals and physical exam.
• Manage patients through acute illness or chronic condition exacerbations.
• Oversee short-term sub-acute rehabilitation episodes.
• Ensure accurate, complete documentation including ICD-10.
• Collaborate with interdisciplinary care team and educate patients/caregivers.
