Job summary

Clover Health is seeking a Nurse Practitioner for our Home Care team to deliver comprehensive in-home primary and supportive care for Medicare Advantage members in Hudson County, NJ. You will conduct in-home assessments, coordinate post-discharge follow-ups, and collaborate with interdisciplinary teams using Clover Assistant to guide care decisions.

Ideal candidates are proactive, technologically adept, and possess an NP license, active DEA/CDS registrations, and a master’s in advanced clinical

Qualifications

• Active NJ RN license required.
• Certified APN (Nurse Practitioner).
• Minimum 1 year NP experience in a clinical setting.
• Active DEA and CDS registrations required.
• Current BLS certification.
• Willingness to travel locally for home visits.

Responsibilities

• Conduct comprehensive in-home health assessments for enrolled Medicare Advantage members.
• Perform post-discharge follow-up visits to reduce hospital readmissions.
• Collaborate with interdisciplinary teams using data-driven clinical tools.
• Proactive, tech-savvy approach to transitional care and supporting decision-making.
