Job summary

Clover Health in New Jersey is seeking a compassionate Nurse Practitioner to join the Home Care Team. The hybrid role combines in-home visits with telehealth care for Medicare Advantage members in Essex County, NJ, building meaningful patient relationships while improving outcomes for medically complex seniors.

The ideal candidate has an NJ NP license, board certification, DEA and CDS registrations, and geriatrics experience; Clover Assistant and EMR tools support proactive care management.

Qualifications

• Active NP license in New Jersey or ability to obtain licensure.
• National board certification as a Nurse Practitioner.
• Active DEA and New Jersey CDS registration, or ability to obtain.
• Experience caring for adult or geriatric populations in primary care or value-based care settings.
• Strong clinical assessment and chronic disease management skills.

Responsibilities

• Conduct in-home and telehealth visits for Medicare Advantage members.
• Perform comprehensive assessments and medication reviews.
• Develop and implement individualized care plans addressing clinical and social determinants.
• Collaborate with physicians, care coordinators, social workers, and team members.
• Monitor health status and intervene to prevent avoidable ER visits and hospitalizations.
