Job summary

Clover Health is seeking a Nurse Practitioner to join our In-Home Care (IHC) Team serving Mercer, Middlesex or Somerset County, NJ. You will deliver longitudinal in-home primary care, focusing on patients with complex chronic conditions and coordinating with providers to improve outcomes.

You will work with clinical autonomy, participate in on-call rotations, and travel locally for home visits while holding an active NJ RN license and APN certification.

Qualifications

• Active NJ Registered Nurse license required.
• Certified Advanced Practice Nurse Practitioner (APN) noted.
• Master’s degree or post-master’s certificate in advanced clinical practice required.
• Minimum 1 year NP experience in clinical setting.
• Active DEA and CDS registrations.
• Basic Life Support (BLS) certification.
• Experience in home care or house call settings preferred.

Responsibilities

• Manage a dedicated patient panel, delivering ongoing in-home primary care.
• Provide care for members with chronic illnesses and advanced medical needs.
• Participate in a shared on-call rotation for continuous support.

This is a unique opportunity to provide comprehensive primary and supportive care directly in the homes of some of our most vulnerable members, helping to improve health outcomes and quality of life.

As a Clover Home Care Nurse Practitioner, you’ll join our In-Home Care (IHC) Team. This team provides longitudinal primary care for members with complex and chronic conditions, focusing on continuity, quality, and outcomes. Clinicians on this team have a high degree of autonomy and play a central role in ongoing care planning.

As a Nurse Practitioner on the IHC Team, you will:

• Manage a dedicated patient panel, delivering ongoing in-home primary care.
• Care for members with chronic illnesses and advanced medical needs.
• Participate in a shared on-call rotation to ensure continuous support.
• Best suited for Nurse Practitioners who value clinical autonomy, excel in building lasting patient relationships, and are skilled in managing medically complex cases throughout the continuum of care.

You Should Get In Touch If

• You are located in or near Mercer, Middlesex or Somerset County, New Jersey and able to travel locally for home visits.
• You have an active and unencumbered NJ Registered Nurse license.
• Are a Certified Advanced Practice Nurse Practitioner (APN).
• Have a master's degree or post-master's certificate in advanced clinical practice.
• Have a minimum of 1 year of NP experience in a clinical setting.
• Have active DEA and CDS registration.
• Have current Basic Life Support (BLS) certification.
• Have experience in home care or house call settings (preferred, but not required).

Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.

A reasonable estimate of the base salary range for this role is: $155,000—$165,000 USD
