Job summary

Optum Home & Community Care in the United States seeks a Nurse Practitioner or Physician Assistant to deliver in-home care across an assigned area. You will perform comprehensive in-home assessments and manage acute or sub-acute conditions with an interdisciplinary team.

You will document thoroughly using ICD-10, provide patient and caregiver education, and travel daily to patient homes. A $10,000 sign-on bonus is available for external candidates.

Qualifications

• Active NP or PA certification (ANCC/AANP or NCCPA) and state licensure.
• Active, unrestricted licensure in practicing state; clean disciplinary record.
• Active driver's license and reliable transportation.
• Current DEA licensure/prescriptive authority or ability to obtain.

Responsibilities

• Conduct comprehensive in-home assessments, including history, vitals and exams.
• Manage acute medical issues and sub-acute episodes in patient homes.
• Document care with ICD-10 coding and coordinate with care teams.
• Provide counseling and education to patients and caregivers.
