Job summary

Signify Health, part of CVS Health, seeks a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for patients in their own homes across licensed states. You will help bridge gaps in care and work with a national clinician network.

The role offers 12 hours per week as a part-time position, with travel to local addresses and occasional virtual visits. Pay ranges from $41.85 to $90.13 per hour, depending on experience and location.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations.
• Engage with patients in their homes and bridging gaps in care.
• Travel locally within licensed states as needed and conduct virtual visits when required.
