Job summary

Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to perform in-home health evaluations. The role offers 29.5 hours per week with travel within licensed states and occasional virtual visits.

Compensation ranges from $37.66 to $81.11 per hour, with benefits for part-time roles. Applicants must hold an active, unrestricted license in coverage areas, board certification, and preferably multi-state licensure.

Responsibilities

• Conduct in-home health evaluations and virtual visits as required.
• Provide Signify Health services including Diagnostic & Preventive Services.
• Collaborate with clinician colleagues to bridge gaps in care.
