Job summary

Optum Home & Community Care is seeking an experienced Family/Adult NP for in-home assessments across Medicare Advantage and other plans. You will work in an interdisciplinary team, conduct comprehensive home visits, and contribute to improving health outcomes by education and collaboration with PCPs.

Flexible scheduling (20–40 hours/week, 8am–8pm window) with no on-call or weekends. A $10,000 sign-on bonus is offered to external candidates.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriactric, or Adult-Gerontology Primary Care.
• Active and unrestricted licensure in the practicing state; ability to obtain by start date.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments during assessments.

Responsibilities

• Conduct comprehensive in-home assessments, including history, meds, vitals, and exam.
• Document diagnoses for care management and planning.
• Share findings with PCPs to address care gaps.
• Identify urgent situations and intervene when needed.
• Educate on diseases, medications, and adherence; address social determinants.
