Job summary

Advantmed is seeking a highly skilled Nurse Practitioner to perform in-home wellness risk adjustment assessments for Medicare members in Saint Charles County, MO. This role emphasizes comprehensive health evaluations, patient education, and closing care gaps, with a flexible PRN schedule.

Providers travel within a 55-mile radius and work independently within our provider network, using EMR systems to document findings and coordinate care.

Qualifications

• Must have valid unencumbered NP license for the state you will be working in.
• Travel within the assigned job posting county up to a maximum 55-mile radius.
• 3 years patient care experience preferred (primary care/adult/geriatric).
• Previous 1099 (PRN) experience is preferred but not required.
• Access to reliable transportation to travel to members’ homes within a designated area.
• Strong ability to work within an EMR system.

Responsibilities

• Perform annual in-home wellness visits and risk adjustment health assessments for Medicare members and other at-risk populations.
• Provide individualized quality care to the elderly population in patients' homes.
• Deliver patient health education on topics such as pain management and medication use.
• Build trusting relationships with members through empathy and communication.
• Assist in closing quality care gaps (screenings and labs).
• Collaborate with care team to enhance delivery of care.
• Commitment to a minimum of 30 hours per month.
