Qualifikationen
- Active and unrestricted NP license in Missouri.
- National board certification applicable to NP specialty.
- Minimum of 3 years direct patient care experience.
- Experience in primary care, family practice, internal medicine, geriatrics, urgent care, or related settings.
- Valid driver’s license and reliable transportation.
Aufgaben
- Travel to members' homes and conduct comprehensive in-home health assessments.
- Perform Annual Wellness Visits and health assessments for members with chronic conditions.
- Document findings accurately and communicate information for post-visit summaries.
- Collaborate with interdisciplinary teams to improve clinical quality measures.
