Job summary

UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (NP) to conduct in-home health assessments for Medicare Advantage and other plan members. The role focuses on education, gap closure, and collaboration with primary care providers to improve outcomes.

The position offers flexible scheduling, a $10,000 sign-on bonus, and 20–40 hours per week with no on-call, weekends, or holidays required.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state (or ability to obtain by start date).
• Driver's license with access to reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• If required by state, eligibility to practice independently and start process within set timelines.
• Preferred: 1+ years of clinical experience in home health or related settings.
• Preferred: Fluent in languages other than English.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plan members.
• Perform medical history review, medication reconciliation, vital signs, and physical exams.
• Provide education on disease processes, medications, and adherence.
• Coordinate with primary care providers to close gaps in care and plan management.
• Identify urgent situations and intervene or refer appropriately.
• Document findings and care plans for care management.
