Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. The role emphasizes education, gap closure, and collaboration with primary care providers, with no on-call, weekends, or holidays required.

Flexible scheduling allows 20–40 hours per week, and the program offers a strong balance of autonomy and teamwork within an interdisciplinary care environment.

Qualifications

• Active certification in Family, Adult, Gerontologic Primary Care as applicable.
• Current NCCPA certification and state licensure (for PAs).
• Active and unrestricted licensure in practicing state with no disciplinary actions.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning; communicate with PCPs.
• Educate members on diseases, medications and adherence; address social determinants of health.
• Identify urgent/emergent situations and intervene appropriately.
