Job summary

Signify Health, part of CVS Health, is hiring a Full Time Clinician (NP or PA) to perform In-Home Health Evaluations, engaging with members in their homes to bridge gaps in care. Visits last about 45 minutes, include a medication and medical history review, a physical evaluation, and optional tests per plan.

You’ll use an iPad with a clinical workflow and are not required to prescribe or alter treatment. Candidates must have active, unrestricted licenses, board certification, and multi-state

• Active, unrestricted licenses in the specified coverage areas.
• Board certification required.
• Multi-state licenses preferred.
