Job summary

Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to perform in-home health evaluations, visiting members in the comfort of their homes and helping bridge gaps in care. You will use an iPad workflow and may conduct virtual visits as needed.

The role requires active licensure, board certification, and flexibility to travel locally within licensed states. Part-time benefits and malpractice coverage are provided as described.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Perform in-home health evaluations (~45 minutes) with patients.
• Engage with patients to bridge gaps in care and provide education.
• Travel locally within licensed states as needed; conduct virtual visits when required.
• Provide Signify Health services including diagnostic and preventive tests (e.g., spirometry, bone density screening).
