Role requires active NP/PA licenses, board certification, and willingness to travel locally and within licensed states. Flexible scheduling and full-time benefits are offered.

Qualifications

• Active, unrestricted license in coverage area(s).
• Board certification is required.
• Multi-state licenses preferred.
• Eligible/qualified as NP or PA.

Responsabilités

• Provide in-home health evaluations to members.
• Review medications and medical history during visits.
• Perform physical evaluations and document findings.
• Coordinate with health plans to bridge gaps in care.
