Job summary

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (NP or PA) to perform In‑Home Health Evaluations. The role involves evaluating patients in their homes, reviewing medications and histories, and determining care needs in coordination with health plans.

The position requires licensure, board certification, and the ability to travel within licensed states, with visits typically lasting around 45 minutes.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
