Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, visiting members in their homes and helping bridge gaps in care. The visit lasts about 45 minutes and includes a health history review, physical evaluation, and optional tests when ordered by the health plan.

The role offers a 40-hour workweek, travel within licensed states as needed, and opportunities for virtual visits.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Bring your heart into every visit, joining a national network of clinicians dedicated to improving health outcomes.
• Visit millions of people nationwide to provide connections to the right care for people’s needs.
• Work with Signify Health as a clinician colleague.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services as indicated (e.g., spirometry, DEE, bone density screening).
