Signify Health, a CVS Health company, seeks a Full Time Clinician (NP or PA) to conduct in-home health evaluations, visiting members in their homes and bridging gaps in care. You will assess medical history, perform physicals, and coordinate with health plans to provide personalized care.

The role requires active license in coverage area(s), board certification, and willingness to travel locally and within licensed states.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Build trusted relationships with health plan members during home visits.
• Conduct in-home health evaluations including med history and physical.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide diagnostic & preventive services as indicated by health plan.
• Use iPad and clinical workflow for each evaluation.
