Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to perform in-home health evaluations. You will visit members at home, review medical history, perform physical evaluations, and support connections to the right care.

Travel locally and within licensed states is required. Required qualifications include an active, unrestricted NP or PA license in the coverage area and board certification; multi-state licenses are preferred.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Join a national network of clinicians delivering in-home health evaluations.
• Engage with members in their homes and bridge gaps in care.
• Follow Signify Health clinical workflow using iPad for evaluations.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Provide Diagnostic & Preventive Services as indicated.
