Job summary

Signify Health, part of CVS Health, seeks a Per Diem Clinician (NP or PA) to perform in-home health evaluations. You will visit members in their homes, review meds and medical history, perform a physical, and coordinate care with the health plan.

This part-time role involves local travel, occasional virtual visits, and a base hourly pay between $37.66 and $81.11. Benefits are available for part-time staff. You must hold an active license, board certification, and may need multi-state credentials.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Join a national network of clinicians delivering in-home health evaluations.
• Visit members in their homes, review meds and medical history, and perform a physical exam.
• Coordinate care by connecting members with appropriate health services.
• Occasionally conduct virtual visits as needed.
• Travel locally and within licensed states when required.
• Provide Signify Health diagnostic and preventive services as ordered.
