Job summary

Signify Health, Inc., part of CVS Health®, seeks a Per Diem Clinician (NP/PA) to provide in-home health evaluations. You will engage with members in their homes, review medical histories, perform brief clinical assessments and coordinate with health plans to connect patients with needed care.

The role offers part-time hours (12 per week), travel within licensed states, and an hourly pay range of $46.03–$99.14 with benefits and malpractice coverage for part-time staff.

Qualifications

• Active, unrestricted license in coverage areas SEATTLE PIERCE.
• Board certification required.
• Multi-state licenses preferred.

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services.
