Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other members. This field-based role emphasizes education, care planning, and improving outcomes through collaboration with PCPs.

You will conduct comprehensive assessments, manage diagnoses, and guide patients on medications and adherence while embracing a flexible schedule and home-based care model.

Qualifications

• 1+ years of clinical experience in family, geriatric, or home health settings.
• Proficiency with electronic medical records and technology.
• Fluent in languages other than English.

Responsibilities

• Conduct comprehensive in-home assessments and collect medical history, vitals, and examinations.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
