Job summary

Optum Home & Community Care is hiring an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

You'll enjoy flexible scheduling (8am–8pm across 7 days) with no on-call, weekends, or holidays, and a path to professional growth within Optum's interdisciplinary care team.

Qualifications

• ANCC or AANP certification for NPs in Family, Adult, Geriatric or Adult-Gerontology Primary Care.
• NCCPA certification and state licensure for PAs.
• Active, unrestricted license to practice in the state of employment, without current disciplinary actions.
• Driver's license and reliable transportation to complete home visits.

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to close gaps in care.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications and adherence.
• Address social determinants of health and provide referrals as needed.
