Job summary

Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to perform in-home health evaluations. Visits last about 45 minutes and include health history review and physical evaluation. You may travel locally and within licensed states, and may also conduct virtual visits as needed.

The role supports connecting patients to the right care and delivering comprehensive, personalized health assessments in their own homes. Benefits are provided for part-time staff.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide In-Home Health Evaluations to members in their homes.
• Engage with patients to bridge gaps in care.
• Use iPad-based workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when necessary.
