Job summary

Signify Health, part of CVS Health, is seeking a Part-Time Clinician (NP/PA) to perform In-Home Health Evaluations, bringing health testing and guidance directly to members in their homes. Qualifications

• Active, unrestricted licenses in coverage areas.
• Board certification required for NP/PA roles.
• Multi-state licenses preferred for broader coverage.

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
