Job summary

Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to perform In-Home Health Evaluations, visiting patients in their homes and helping bridge gaps in care.

Visits last about 45 minutes, include a medication and medical history review, a physical evaluation, and optional diagnostic tests as needed. Part-time role with travel within licensed states, occasional virtual visits, and a broad benefits package.

Qualifications

• Active, unrestricted licenses in the coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations at patients' residences.
