Job summary

Signify Health, part of CVS Health, seeks a Per Diem Clinician (NP/PA) to provide in-home health evaluations. You will visit patients in their homes, review medical history, perform physical evaluations, and connect them with the right care.

Flexible to travel locally within licensed states, with part-time hours and a base hourly rate plus incentives. Malpractice insurance is provided; supplies and other perks are included.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations for CVS Health members.
• Bridge gaps in care by engaging with patients and providing guidance.
• Travel locally to member homes as needed and perform virtual visits when required.
