Qualifications

• Active, unrestricted license in coverage area(s) required.
• Board certification required for NP/PA.
• Multi-state licenses preferred.

Responsibilities

• Provide In-Home Health Evaluations.
• Engage with members to review medications and medical history.
• Perform physical evaluations and coordinate with health plans for additional tests.
• Travel locally and within licensed states; conduct virtual visits as needed.
