Job summary

Signify Health, part of CVS Health, seeks a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. You will engage with members in their homes, review medical history, perform a physical exam, and connect patients with appropriate care.

The role requires licensure in target states, board certification, and the ability to travel locally. This part-time position offers flexible hours and a supportive team focused on improving health outcomes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Engage with members in their homes to support health evaluations.
• Perform comprehensive health evaluations including medical history and physical assessment.
• Coordinate with health plans to connect members with appropriate care.
• Travel locally within licensed states as needed to complete evaluations.
• Conduct virtual visits when required by the plan or member location.
• Provide diagnostic and preventive services as indicated by plan guidelines.
