Job summary

Signify Health, part of CVS Health, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations, visiting members in their own homes and bridging gaps in care.

The visit lasts about 45 minutes and includes history review and physical examination with optional tests; use an iPad and a clinical workflow. You will travel locally and within licensed states, with a 29.5-hour workweek.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).
• Able to cover the following primary locations: Otero County

Responsibilities

• Join a national network of clinicians dedicated to improving health outcomes.
• Visit members in their homes to conduct in-home health evaluations (approx. 45 minutes).
• Perform medication and medical history review, physical evaluation, and optional diagnostic tests as ordered by the health plan.
• Use an iPad and clinical workflow; do not prescribe medicine, order labs, or alter current treatment.
• Be flexible to travel locally and within licensed states; conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services as indicated.
