Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for plan members in people’s homes. The visit lasts about 45 minutes, including history, evaluation, and plan-related steps, with an iPad workflow.

This role offers about 29.5 hours weekly, potential for virtual visits, and travel within licensed states. A strong clinical background and board certification are required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening).
