Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. You will visit members in their own homes, perform medication and medical history reviews, and deliver a personalized assessment that helps bridge gaps in care.

The role features roughly 29.5 hours per week, local travel within licensed states, optional virtual visits, and a benefits package for part‑time employees.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi‑state licenses are preferred.

Responsibilities
