Job summary

Signify Health, part of CVS Health, seeks a Part-time Clinician (NP or PA) to perform in-home health evaluations for health plan members in their homes, using an iPad and clinical workflow.

Each 45-minute visit includes a medication/history review and physical evaluation; tests may be added per plan. Requires active licenses, board certification, and ability to travel regionally within licensed states. Virtual visits possible.

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi‑state licenses (preferred)
• Able to cover primary locations: Otero County

Responsibilities

• Provide in-home health evaluations to health plan members.
• Engage with patients in the comfort of their homes and bridge gaps in care.
• Use iPad and clinical workflow during evaluations; no need to prescribe or order labs.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Perform Diagnostic & Preventive Services per health plan as ordered.
