Job summary

Signify Health, Inc. is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations for health plan members, typically about 45 minutes per visit. You’ll review medications, medical history, perform a physical evaluation, and, if ordered, additional diagnostic tests in the home.

Visits are conducted nationwide, with some virtual visits as needed. This role requires an active license and board certification, and involves travel within licensed
