Job summary

Signify Health, Inc. is seeking a Part-Time Clinician (NP or PA) to perform in-home health evaluations, visiting members in the comfort of their homes. The role supports bridge-building for better care access and health outcomes.

The clinician will perform comprehensive assessments, including medication and history reviews, and may be asked to conduct tests as per health plan orders. Travel locally and within licensed states is required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).

Responsibilities

• Engage with health plan members in their homes during in-home health evaluations.
• Conduct medication and medical history review, physical evaluation, and optional diagnostic tests as ordered by health plan.
• Build trusted relationships to improve health outcomes and connect members to right care.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
