Signify Health is seeking a part-time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations. The role involves visiting members at their homes, reviewing medical history, performing a physical evaluation, and scheduling necessary tests as directed by health plans.

The clinician will travel locally and within licensed states, using an iPad for the visit workflow. Applicants must have active, unrestricted licenses and board certification; multi-state licenses

Qualifikationen

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.
• VA residence is preferred.

Aufgaben

• Provide in-home health evaluations and engage with members in their homes.
• Utilize iPad and clinical workflow to conduct visits (~45 minutes each).
• Coordinate care and connect members with appropriate services.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Adhere to Signify Health services including diagnostic and preventive measures.
