Job summary

Signify Health, Inc. is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. The clinician will engage with patients at their homes, review medications and medical history, and conduct physical evaluations using an iPad and clinical workflow.

The role requires NP/PA licensure, board certification, and flexibility to travel locally and within licensed states. Part-time benefits apply, including malpractice coverage and supplies.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required for NP/PA practice.

Responsibilities

• Come to patients' homes for in‑home health evaluations.
• Engage with patients to review medications and medical history.
• Perform physical evaluations and discuss results with patients.
• Coordinate with health plans for additional tests if ordered.
• Conduct virtual visits when needed.
• Provide Signify Health services including Diagnostic & Preventive Services.
