Signify Health, Inc. is seeking a Part-time Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for members in their own homes nationwide.

The visit lasts about 45 minutes and includes a medication and medical history review, physical evaluation, and optional tests as ordered by the health plan. You may travel locally and within licensed states, with virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
