Job summary

Monogram Health Professional Services seeks an Advanced Practice Provider (NP/PA) to deliver in-home, evidence-based care for patients with multiple chronic conditions. You will work with a multidisciplinary team, conducting wellness and acute visits, and coordinating with PCPs and specialists to optimize outcomes.

The role includes extensive travel to patient homes, mileage reimbursement, and opportunities for CEU/CME reimbursement, with a total rewards package that includes a base salary,

Qualifications

• Active and unrestricted RN license.
• Board certification for NP or PA.
• Current and unrestricted DEA certificate or eligibility.
• Minimum of two years of direct patient care experience.

Responsibilities

• Provide in-home patient care to complex cases.
• Perform comprehensive assessments and wellness visits.
• Educate patients and caregivers on disease management and preventive care.
• Participate in virtual integrated care team meetings.
• Coordinate care with PCPs and specialists.
