Job summary

Porter is expanding its services and seeks Nurse Practitioners to support patients across New Jersey in a state-wide traveling position. The role involves conducting in-home health risk assessments and collaborating with Porter Care Guides to provide comprehensive care management.

The ideal candidate will have a valid New Jersey NP license, a strong sense of empathy, and at least three years of patient care experience. Competitive wages, flexible schedules, and travel accommodations are offered.

Qualifications

• 3 years of patient care experience required, ideally in primary care/adult/geriatric.
• Exceptional communication skills to assist patients and coworkers with sincerity.
• Valid unencumbered New Jersey NP license is mandatory.

Responsibilities

• Conduct in-home health risk assessments for members.
• Perform evaluations, medication reconciliations, and comprehensive physical exams.
• Identify diagnoses for care management and treatment.
