Job summary

Signify Health, part of CVS Health, is hiring Per Diem Clinician (NP or PA) to perform in-home health evaluations. You will visit members in their homes, complete health questions, med histories, and physicals, and coordinate with the health plan as needed.

The role includes local travel and possible virtual visits. The position offers part-time hours (12/week) with a pay range of $46.03–$99.14 per hour and comprehensive benefits for part-time staff.

Qualifications

• Active, unrestricted license(s) in coverage area(s) KENNEWICK ADAMS.
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Conduct in-home health evaluations lasting about 45 minutes.
• Perform medication and medical history review and physical evaluation.
• Utilize a mobile workflow on an iPad to document visits and coordinate with health plans.
• Travel locally and within licensed states as needed; conduct virtual visits when required.
